Personal social security proxy model

When the insured person goes to the social security center to apply for the social security transfer certificate, he/she needs to provide his/her ID card and photocopy, fill in the social security transfer application form and entrust others to do it. At the same time, I need to provide my ID card and copy and power of attorney (notarized and sealed).

If you want to entrust others to handle the social security transfer voucher, you can write like this:

XXX Municipal Human Resources and Social Security Bureau:

I am xxx, male, born on xX, xx, and my registered address is xxX, XX Town, XX District, XX City, XX Province. As I can't personally handle the inter-provincial (city) transfer voucher of social security, I hereby entrust someone with the ID number (123456789123456789) to handle it on my behalf.

After writing the power of attorney, go to the notary office for notarization and seal. Give it to the customer.
