Social Security has one card and one card

There can only be one social security card.

I. Social security card processing materials:

(a) a completed application form for registration of social security card issuance;

(b) a description of the preparations for the issuance of social security cards;

(c) application areas and the structure of the card application file in line with the requirements of the unified national norms (including locally expanded application areas and indicators);

(d) Card face style designed in accordance with the unified requirements;

(v) local social security card management measures and implementation rules;

(vi) other materials required by the Ministry of Human Resources and Social Security.

The process of social security card application:

(1) Employees applying for the card for the first time go to the second-generation ID card networked photo studio recognized by the municipal public security organs to take a digital photo, and ask the studio for the "XX Municipal Social Security Card Digital Photo Reply Slip", and fill in the name and ID card number on the Reply Slip neatly, in order to facilitate the application. Employees who have already applied for a social security card but have lost it and need to reapply do not need to submit the digital photo receipt.

(2) Enterprises that have applied for online filing can directly apply for a license online. First of all, print the list of card production, will need to make the card employees digital photo receipt (lost card replacement does not need this receipt) according to the printed list of card production in the order of the personnel and attached to the list, and provide a copy of the employee's ID card, submitted to the social security designated card window, pay the cost of the card, get the card production receipt.

(3) If the online declaration is not done, the enterprise submits a copy of the employee's ID card and the receipt of the digital photo of the social security card (this receipt is not required for the replacement of the lost card), and the receipt of the digital photo of the social security card should be arranged in the order of the participation statement and attached to the statement of participation. If you lose your social security card and need to reapply for it, you can directly fill in the "x City Social Security Card Replacement (Lost) Application Form". After printing the list of card production by the collection department to which the contribution belongs, you can go to the designated card production window of the social security to pay the labor cost and receive the card production receipt.

(4) For those who pay at the individual payment window, they should submit their original ID card and the digital photo receipt of the social security card (this is not required for those who have already applied for a social security card) to fill in the form and pay the labor cost at the individual payment window of the social security institution to which they belong, and receive the card production receipt[2].
