The Xi'an Maternity Insurance Reimbursement Regulations stipulate that the spouse of a male employee who participates in maternity insurance belongs to a non-urban employee who has not participated in maternity insurance, and will be given a one-time subsidy by the Maternity Insurance Fund according to 50% of the subsidized standard of medical fees for female employees' hospitalized delivery, which is 4,000 yuan for cesarean section and 2,500 yuan for vaginal delivery.
Xi'an Maternity Insurance Policy
I. Maternity insurance treatment reimbursement items:
Cesarean section; normal delivery; abortion or termination of pregnancy in 3-7 months; abortion or termination of pregnancy in less than 3 months; IUD or removal of the IUD; sterilization surgery, vasectomy (oviduct) reversal; conservative (surgical) treatment of ectopic pregnancy.
II. Documents required for declaration of maternity insurance subsidy:
1. Female workers are required to provide the following documents when declaring maternity:
1). Application form for payment of maternity insurance benefits for employees in Xi'an;
2). Hospitalization invoice and outpatient bills; (i.e., hospitalization fee and prenatal examination fee);
3). Copy of birth certificate and medical certificate of the baby's birth;
4). A copy of the employee's ID card; a copy of the marriage certificate;
5). A copy of maternity medical record, outpatient expense list, first page of hospitalized case, long-term medical advice, temporary medical advice, and detailed list of medical expenses; (the underlined part is copied at the hospital);
6). A copy of the Chinese bank card and ID card in which the reimbursement is opened in the reimbursement person's own name (copied on the same piece of paper).
2. The spouse of a male employee is also required to provide when declaring the birth:
1). A certificate of no work provided by the spouse; (produced by the township or street where he/she lives)
2). A copy of the spouse's ID card;
Note: The maternity reimbursement program for the spouse of a male employee only includes normal delivery and cesarean section, and other programs will not be reimbursed.
3. Family planning surgery and ectopic pregnancy declaration of materials to be provided:
Xi'an City workers maternity insurance treatment payment application form, hospital bills, medical expenses, diagnostic certificates, a copy of ID card, a copy of the marriage certificate, outpatient medical records and inspection, laboratory reports; if hospitalization costs, with the need to provide the first page of hospitalization cases, long-term medical advice, temporary medical advice. 1. Time limit for reimbursement:
Participating employees who have completed maternity leave, artificial termination of pregnancy or family planning surgery (IUD, IUD removal, abortion, ectopic pregnancy, sterilization, vasectomy (oviductal) reversal) within 90 days after delivery, the insured unit operator unified reporting, individual reporting will not be accepted.
2. Conditions to be fulfilled for reporting:
(1). The employee's unit to participate in maternity insurance and pay contributions on time;
(2). Comply with national, provincial and municipal family planning policies;
(3). Giving birth and performing artificial termination of pregnancy at designated medical institutions, as well as practicing family planning surgery at designated family planning technical service institutions.
Xi'an Maternity Insurance Reimbursement Procedure
(1) ID card of the person (with a copy of the same) and a certificate of the unit (proving that the participant is an employee of the unit, participated in the maternity insurance for the employees of the Economic and Technological Development Zone, and is in line with the national family planning policy)
(2) Maternity insurance cost information (including medical bills and copies, cost lists, medical diagnostic certificates, and the first page of the hospitalization medical records, Discharge Record, Hospitalization Medical Record, Long-term Medical Orders, Temporary Medical Orders);
(3) Proof of planned childbirth issued by the population and family planning management department of the district or county or above;
(4) If you declare the cost of family planning, you are also required to provide a certificate of long-lasting birth control surgery in Xi'an City, China, or a medical certificate and medical information issued by a designated medical institution (or family planning service institution).
(5) In case of delivery or artificial termination of pregnancy in a non-designated medical institution due to an emergency, it is also necessary to provide a certificate of diagnosis of the emergency; and a written certificate of belonging to the designated hospital of the local health insurance and the level of the hospital is provided by the hospital of attendance. If the operator entrusts another person to handle the application, a power of attorney should be issued and stamped with the official seal of the organization. The declared information will not be returned, if there is other use, please make a backup in advance. Those who submit information beyond the time limit will be responsible for their own costs.
7. If the spouse of a male employee who has taken part in maternity insurance does not take part in maternity insurance, he or she can be subsidized according to 50% of the subsidy standard. A copy of the marriage certificate, a copy of the woman's ID card, and a certificate of no fixed occupation issued by the community or village committee where the woman's household is located are also required for filing.

9. Medical expenses incurred as a result of a medical accident during the employee's childbirth or family planning surgery are not included in the scope of payment by the Maternity Insurance Fund.
10. The designated medical institutions for employees' maternity insurance are the same as the current designated medical institutions for employees' basic medical insurance.
