Chongqing medical insurance personal account entry standard:

Employees under/kloc-0 and under 35 years of age are included according to 1 .3% of their basic medical insurance payment base;

2, 35 to 44 years old workers, according to my basic medical insurance payment base 1.5% included;

3, workers over 45 years old, according to my basic medical insurance payment base 1.7% included;

4, retirees according to the unit on-the-job workers per capita payment base of four percent included. For employers with only retirees and no employees on the job, the personal accounts of retirees were included in 4% of the social wages of our city in the previous year;

5. Retirees from state-owned closed and bankrupt enterprises and older laid-off workers from state-owned enterprises pay their medical expenses for the rest of their lives in one lump sum, which are included in personal accounts according to 4% of the average salary of our city last year.

Insurance procedures:

1, the insured unit or individual shall truthfully fill in the Registration Form of the Insured Unit and the Registration Form of the Insured Person, and provide a copy of the legal person business license, organization code certificate, tax registration certificate and payroll;

2, medical insurance office in accordance with the provisions, review the relevant information, the qualified insured information input computer management, print "payment notice" and "collection plan list";

3, the insured units or individuals pay large medical insurance premiums, card fees, and pay two 1 inch bareheaded photos, and at the same time make medical insurance "special medical record" and IC card.

The basic medical insurance for employees, the new rural cooperative medical system and the basic medical insurance for urban residents shall be implemented in accordance with state regulations.
