China Bank Shanxi Financial Social Security Card (Social Security Card) Replacement Process

The social security card can only be reissued after the bank officially reported the loss. The social security card can be replaced if it is damaged, demagnetized or exceeds its service life.

The specific procedures are as follows:

1. You need to bring your original valid ID card and a copy of your ID card (if you have an agent, you should also bring your original ID card and a copy of your agent's ID card) to the social security card comprehensive service window for card replacement and replacement procedures;

2. When going through the formalities of card replacement and replacement, you must fill in the "Shanxi Social Security Card Business Processing Form" and issue the original valid identity document.

3. After the social security card comprehensive service window accepts the application for replacement card, you pay the corresponding social security card production fee (28 yuan) (collected by the social security card comprehensive service agency), and then receive the payment voucher after payment.

4. Go through the confirmation procedures for the replacement card and issue the "Shanxi Social Security Card Acceptance Receipt".

5. After 30 working days, I will go to the China Bank window where the social security card was originally opened with my original valid ID card and the receipt for handling the social security card business in Shanxi Province (if you need to report the loss, you need to hold an application for reporting the loss of individual customers).

Tip: If the cardholder has already applied for a replacement card, even if the original card is retrieved, the original card can no longer be used. After completing the card replacement procedures, the social security card comprehensive service window will no longer accept the cancellation and loss reporting procedures.
