The process of filling in medical insurance card information online:

1. Application conditions and materials: 1. 3. Login to the online social security card application website: enter the login interface, select the corresponding options of business type, business object, related business and inquiry processing, enter the correct personal ID number and name option, and click Login to enter the application system;

4. Personal information verification: Click the menu on the left: (Step 1) Personal information verification, and fill in personal information, with an * as a required item. If there is no information about the public security photographer, you should manually enter the "photo receipt" item.

If the photo receipt is obtained from the photo library according to the social security card photos, if the police photos already exist, you don't need to enter the photo receipt, and the system has defaulted. After filling it out, check it and click Submit.

5. Print the declaration form: the page will automatically jump to "(Step 2) Print the declaration registration form", and click Download Registration Form or Print Registration Form: Download the registration form to generate the word document of the Registration Form for Social Security Card Application of the People's Republic of China, which will be printed and used in the printing shop; Printing the registration form means printing the Registration Form for Social Security Card Application of the People's Republic of China directly on the webpage, which is suitable for users with printers;

6. Payment book printing: the page will automatically jump to (step 3) payment book printing, and click Download Payment Book or Print Payment Book: Download Payment Book means to download and generate the word document of Non-tax Income Payment Notice for printing in the printing shop; Printing payment book refers to printing non-tax income payment notice directly on the web page, which is suitable for users with printers;

7. Personal payment: The whole process of personal online application is completed after the payment form is printed. Applicants must hold the Notice of Payment of Non-tax Income to ICBC, Agricultural Bank of China or China Construction Bank and obtain a financial receipt before they can wait for the notice.

8. Processing time and expenses: The processing time of social security card is 20 working days after successful payment, and the expense is 20 yuan/card. You can pay the fees at the outlets of ICBC, Agricultural Bank of China and China Construction Bank with the Notice of Payment for Non-tax Income, and get the financial receipt.

9. Information required for card application: 1. Original ID card. If there is no ID card, provide the original household registration book (if there is no ID card, provide the original ID card of the agent); Then there is the Registration Form for Social Security Card Application of the People's Republic of China.
