Primary school students in Changsha apply for medical insurance cards. The steps are as follows:

1. Provide Changsha Children's Medical Insurance Registration Form for students in the park.

2. The original children's household registration book or ID card, and the relationship certificate between the insured and the guardian or the original children's birth certificate and provide a copy.

3. The digital photo receipt of the photo point of the second-generation ID card recognized by Changsha Public Security Bureau, and indicate the name and ID number on the receipt (children under 4 years old do not need to provide it) and provide a copy.

4. The original bank passbook of the guardian, and provide a copy.

5. The original guardian's household registration book or the original valid certificate of foreigners from Hong Kong, Macao and Taiwan and provide a copy.

6. If the guardian is a soldier, he/she shall examine the valid military officer's card and provide a copy, and provide the certificate of troops stationed in Shenzhen.

7. Non-Changsha household registration students provide the original family planning certificate of Changsha.
