Materials required:
1. If the insured female employee gives birth to the first child (the first abortion after marriage is also counted), the following documents are required (depending on the region, copies can be attached):

(10) If it is induced abortion, you need to bring the perinatal outpatient medical records, doctor's orders, medical expenses list and family planning certificate issued by the family planning department;
(1 1) If both parents are only children and neither husband nor wife has an only child certificate, both parents' only child certificates are required.
2. If the spouse of the insured male employee has no work unit, he/she shall carry the above certificates, and:
(1) marriage certificate
(2) Identity cards of both parties
(3) the woman's household registration book (if the household registration book does not indicate whether it is an agricultural population, it is necessary to provide proof of the nature of the household registration issued by the local household registration department)
(4) If the spouse is registered in the city, an unemployment certificate shall be provided; If it is agricultural registered permanent residence, the unemployment certificate of the village committee should be provided.
3. If it is handled by the enterprise manager, it is also necessary to provide:
(1) General tax payment for the current month (work injury, maternity)
(2) Payment voucher for work-related injury and maternity arrears
(3) Social insurance statement of the current month.
