After the social security card is lost or stolen, the insured can report the loss to the provincial medical insurance bureau and the business outlets of China Construction Bank respectively. You can report the loss orally by telephone, or in writing at the social security card service window of the Provincial Medical Insurance Bureau and any business outlets of China Construction Bank. You can only apply for a replacement card after reporting the loss in writing.

Oral loss reporting process: Call the social security card service telephone number 0971-8213983 of the Provincial Medical Insurance Bureau and the customer service telephone number 95533 of the Construction Bank to report the loss orally for medical insurance function application and financial function application. The telephone should provide the lost person's name, ID number, insured unit and other related information.

Written loss reporting and card replacement procedures:

1. Cardholders need to bring the original and photocopy of their ID cards to the social security card service window of the Provincial Medical Insurance Bureau to report the loss and reissue the social security card, and then go to any business outlet of China Construction Bank to report the loss of the financial function (whether the financial function is enabled or not, they must report the loss and cancel the financial function of the card), and pay per card according to the charging standard published by China Construction Bank 10.

2. If someone else is entrusted to handle the loss reporting and follow-up business, the entrusted agent shall carry the receipt for the loss reporting of social security card business issued by the Provincial Medical Insurance Bureau, and go to any business outlet of China Construction Bank to handle the loss reporting of financial functions according to regulations.

3. 60 days after completing the formalities of reporting the loss and replacing the card, I or my agent will bring the original ID cards of both parties and the receipt of the social security card to the provincial medical insurance bureau to get a new card.

Procedures for canceling loss reporting: You can't cancel the loss reporting after handling the written loss reporting, and only the person who handles the oral loss reporting can cancel the loss reporting. If the insured person needs to report the loss orally, the cardholder shall carry the social security card and valid identity documents to the social security card service window of the Provincial Medical Insurance Bureau and the business outlets of China Construction Bank respectively.
