Required materials for social security card unlinking

Adults:

1. In person: original ID card, original social security card

3. Minors: ID card for those who have already applied for an ID card; original household register or certificate of household registration for those who have not yet applied for an ID card, original original social security card, original guardian ID card, and original proof of guardianship.

4. If a guardian entrusts another person to handle the application for a minor: if he/she has already applied for an ID card, he/she is required to produce an ID card; if he/she has not applied for an ID card, he/she is required to produce the original household register or certificate of household registration, the original social security card, the original ID card of the guardian, the original certificate of guardianship, the original written letter of authorization signed or sealed by the guardian, and the original ID card of the person who is acting on behalf of the guardian.

Can a lost and found social security card be used

Whether a lost and found old social security card can be used depends on whether the cardholder has only gone through the procedure of losing the card at the bank or has gone through both the procedure of losing the card and the procedure of replacing it. If the card was only lost, you can go to the bank to complete the procedure to unregister it, so that the old social security card can continue to be used. If you lose your card and then replace it, you can't use your old card.

Social insurance agencies shall record in a timely, complete and accurate manner the contributions made by individuals participating in social insurance and by employers on their behalf, as well as records of their individual rights and interests, such as the enjoyment of social insurance benefits, and shall regularly send the record sheets of individual rights and interests to the individuals free of charge.

Employers and individuals can inquire and check their records of contributions and social insurance entitlements with the social insurance administration organization free of charge, and request the social insurance administration organization to provide social insurance consulting and other related services.
