It should be noted that the "Application Form for Transfer of Social Insurance Relationship" must be issued by the social insurance agency in the place where the social insurance relationship is transferred and stamped with the official seal. When filling in the form, it is necessary to ensure the accuracy of the bank, account name, account number and other information transferred to the social insurance agency.

When an employee changes his/her work unit in the same overall planning area, the unit does not need to apply for the transfer of the employee's social insurance relationship, but only needs to stop the insurance from the transfer-out unit, and the transfer-in unit can start to participate in the insurance the next month after the employee stops the insurance.

On the other hand, when the unit handles the connection of social insurance relationship for the transferred employees, it should provide the social insurance agency with the inquiry form for the transfer of social insurance relationship of employees and the employee endowment insurance manual of the original insured unit, and indicate the unit code, contact person and contact telephone number on the inquiry form (original).
