Lost social security card

There are three ways to report the loss in advance:

(1) You can call the social security card service hotline "96102" (24-hour service) to report the loss in advance;

(2) You can report the loss in advance in writing at the social security card service outlets with your ID card;

(3) Self-service pre-loss reporting is carried out on the self-service terminal of social security card set by social security card service outlets or designated medical institutions.

Materials:

1. If the social security card is lost, stolen or damaged, it shall be reported to the nearest social security agency or reissued with the ID card (or household registration book);

2, the original medical insurance IC card for social security card, but also need to provide a one-inch color photo (based on the second-generation ID card photo);

3. If you entrust others to handle it, you should bring the ID card (or household registration book) of the principal and agent;

4. The expenses incurred during the loss or damage of the social security card shall be reimbursed at one time after examination, which is the latest medical expenses incurred before the loss is reported for replacement.

Process:

1, call 962222 to report the loss (note: 1, the loss can only be reported within 24 hours, so once the loss is reported, it must be reissued within 24 hours, otherwise the lost card will be valid again;

2. Go through the formalities at the social security service center;

3. Handle on the spot and get the certificate on the spot;

Article 3 The social insurance system adheres to the principles of wide coverage, basic protection, multi-level and sustainability, and the level of social insurance should be compatible with the level of economic and social development.
