If the social security card (citizen card) is lost or damaged due to improper use and storage, you should go to the medical insurance management center to change the card with relevant materials in time.

I. Charging Standard According to the Reply of Fujian Provincial Department of Finance and Fujian Provincial Price Bureau on Relevant Issues Concerning Charging Social Security Cards in Fujian Province, if the social security card is damaged or lost, the charging standard is 25 yuan/card.

1, I'll handle it. Provide the original and photocopy of my valid ID card (two copies).

2. Others handle it. Under special circumstances (such as mobility difficulties, old age, etc.), you can entrust others to handle it on your behalf.
