1. The applicant or agent fills in the Application Form for Personal Business of Social Security Card in Zhongshan City, Guangdong Province at the service bank's handling outlets and signs it for confirmation;
2. 4. Precautions:

(3) Units applying for business card printing must receive cards in a unified way, and may not receive cards separately in their own names;
(4) Photos of applicants with local household registration and second-generation ID cards can be directly extracted from the public security photo library without taking digital photos. (subject to the update of the public security photo gallery, if the second-generation ID card has just been issued less than one year, the public security photo gallery may still not be updated, or digital photos are needed. You can first check whether there are photos in the public security photo library at the handling outlets).
