How to apply for social security after renaming?

Hello: To modify the name of the social security person, I or the social security agent of the insured unit can bring the following information to the collection department of the social security institution in my district to apply for change: (1) Application Form for Change of Social Insurance Participation Information (I don't need to fill in this item if I am not employed in the unit); (2) Second-generation ID card (check the original and keep a copy); (3) Description of my situation (sign my full name and press my thumb).

If the name of the insured information changes or the age of the ID card number changes, it should also submit the certificate of correction of the duplicate number and wrong number of the citizenship number issued by the public security department where the household registration is located (stamped with the special seal for household registration).
