After the insured person has paid the maternity insurance premium in full 12 months, it complies with the family planning policy, marriage law and other relevant provisions. However, within 90 days from the date of birth, the employer or I shall bring relevant information to the social security agency of the insured place for reimbursement (in the state of insurance payment at the time of reimbursement).

Maternity insurance reimbursement needs to provide the following materials: fill in the Approval Form for Maternity Insurance Benefits in Chengdu in duplicate (with the official seal of the company), ID card, original hospitalization expense document, discharge certificate, birth index, baby birth certificate or other medical certificates, marriage certificate and personal settlement passbook (individuals need to provide).

If a child is born in a different place, it is also necessary to provide the grade certificate of the medical institution for birth in a different place and affix the official seal of the hospital (you can log in to the website of Chengdu Human Resources and Social Security Bureau to download the form or collect it at the corresponding window of the agency in the insured place). Children with birth indicators in different places must go to the household first, and a child account should be issued when reimbursement is made. If the indicators are qualified, the household registration book should be stamped on the copy.
