1, before the flow, I or the original employment unit shall apply to the original insurance agency, and the original insurance agency shall generate an insurance certificate in triplicate. The personnel transferred from our city need to bring the original and photocopy of the employee's basic medical insurance card, medical insurance card and ID card, and provide the original and photocopy of the agent's ID card to apply for issuing the insurance certificate.

2, migrant workers or new employment units should be within 3 months after the termination of the original basic medical insurance relationship to the new employment agencies for insurance procedures. After accepting the application, the new agency will handle the insurance procedures for applicants who meet the local insurance conditions, and contact the original agency to generate and issue a "contact letter";

3. After receiving the Contact Letter, the original insurance agency fills in the Information Form together with the Insurance Certificate and submits it to the new employment agency;

4.
