When an enterprise applies for insurance for the first time, it must bring/kloc-0 copies of its business license, organization code certificate, local tax registration certificate, legal person ID card, basic bank account number and account opening permit, and fill in the Social Insurance Registration Form for Employers (in quadruplicate) to register with the social security center. At the same time, fill in the roster of insured persons (name, ID number, contact number, month of insurance) and report it to the collection department of social security center. After being audited by the social security center, the basic medical insurance premium shall be declared and paid to the local tax department of the enterprise before the next month 10.
2, urban individual industrial and commercial households (freelancers) to apply for insurance, must carry a business license (freelancers with a copy of their household registration book), two copies of my ID card, three one-inch photos, to the social security center for insurance procedures, the establishment of social insurance relations (floating population must carry the local social security department for the first time not insured certificate).
3. When applying for insurance, the unemployed in an enterprise shall bring the Manual of Medical Insurance, the Notice on the Calculation of Renewal and Cohesive Payment Period of Medical Insurance Premium issued by the enterprise, and the documents on the termination of the labor contract to the social security center for renewal procedures.
