(2) Application for maternity allowance and nursing allowance

After a female worker gives birth or miscarries, the unit shall apply for maternity allowance for her.

The unit shall fill in the Application and Review Form for Maternity Allowance and Nursing Leave Allowance in Wuhan (Table 6) in duplicate, affix the official seal of the unit, and attach the approved Form 1 (one copy), discharge summary and copy of birth certificate, and report it to the window on the second floor for bookkeeping;
