Information required for reimbursement of hospitalization in different places:

2. Detailed list of expenses (original 1 copy);

3, outpatient medical records (copy 1, the original inspection);

4. Inpatient medical records stamped with the official seal of the medical institution (need to be copied in the hospital medical record room: admission records, doctor's orders, operation records, discharge records and related inspection reports) (copy1);

6. Social security card of the insured (copy 1, original inspection);
