Before and after the full launch of the work, the Municipal Information Service Office and the Municipal Medical Insurance Bureau have issued a series of regulations, in order to facilitate the management and operation of the relevant units, the main contents of the regulations are summarized below, and the relevant units are requested to comply with the implementation. I. Application and Issuance of Social Security Cards 1. Targets of Social Security Card Application According to the Measures for the Administration of Shanghai Social Security Cards (Municipal Government Decree No. 100), citizens who have reached the age of sixteen and have permanent residence in the city can apply for a social security card. At this stage, in addition to continuing the work of applying for social security cards for the city's medical insurance participants, the work of applying for social security cards for other urban residents with permanent residence in the city who have reached the age of sixteen has begun one by one. 2. Procedures for applying for social security cards Any citizen who meets the conditions for applying for a social security card may apply for one with his/her household registration book, his/her ID card and his/her spouse's ID card at the street (township) of his/her place of residence or at the nearest street (township) social security card service station. If, during the application process, the applicant finds that the information on his or her name and civil status number recorded in the public security or labor security departments is inconsistent, or differs from the actual use of the card, he or she should first apply for correction of the information, and then go through the procedure of registering to apply for a social security card. 3. Receiving a Social Security Card After a citizen applies for a social security card, the Municipal Information Service Center will issue a social security card within 30 days from the date of acceptance of the application (except for those who applied before the end of 2001 and those whose information is inconsistent). Citizens can receive their social security cards at the place where they originally registered to apply for a social security card. When receiving a social security card, you are required to fill out a detailed "Receipt for Receipt of Social Security Card" and sign the receipt. 4. Loss of Social Security Card If a cardholder finds that his/her social security card has been lost or stolen, he/she can lose it by phone through the hotline (962222) set up by the Municipal Information Service Center or in writing at the district or county social security card replacement (replacement) service outlets. If the cardholder loses his/her card by phone, he/she should lose it in writing within 24 hours after he/she loses his/her card by phone. The written loss is valid for 30 days, and the lost cardholder should make the choice of lifting the loss or reclaiming the social security card within this 30-day period. The City's Information Service Center will seal the lost card and stop the use of the card within four hours of receiving confirmation of the loss by telephone or accepting the written loss. Any loss to the individual or the social insurance fund caused during the period of time in which the card is not in effect will be the responsibility of the person who lost it. 5. Replacement (replacement) of social security cards After the social security card is damaged or lost, the cardholder can apply in writing for a replacement (replacement) of the card at the nearest district or county social security card replacement (replacement) card service center with his/her account book (the agent with his/her social security card or account book and the applicant's account book), and the staff will check that the information is correct and confirm that there is such person's information in the database of the Municipal Social Security Card Service Center, and review the status of the social security card with that of the applicant. After confirming the information of the person in the database of the Municipal Social Security Card Service Center, and reviewing the status of the social security card and the applicant, the staff will collect the cost of the card from the applicant in accordance with the regulations and issue a special receipt. After completing the application procedures for the replacement (exchange) card at the district and county social security card replacement (exchange) card service outlets, the applicant can receive the card after 15 working days from the date of completing the procedures by presenting his/her household registration booklet and the "Shanghai Social Security Card Replacement (Exchange) Card Application Form". When the new social security card is issued, the staff at the service point will hand it over to the applicant for confirmation and signing. II. Application and Issuance of Medical Insurance Card 1. Subjects and Methods of Issuance of Medical Insurance Card Subjects who are not social security card applicants will be issued with medical insurance cards by the municipal, district and county medical insurance centers under the Municipal Bureau of Medical Insurance through the units in which they are located after they have participated in the basic medical insurance of the city. 2. Then, they should go to the nearest municipal or county health insurance service center to apply for a temporary health insurance card by presenting their ID cards and the Application Form for Social Security Card (for Medical Insurance). Use of Social Security Card and Medical Insurance Card 1. The social security card or medical insurance card held by basic medical insurance participants can be used for medical treatment at designated medical institutions, purchasing medicines at designated retail pharmacies, and going to district and county medical insurance affairs centers to handle medical insurance affairs. For the time being, the social security cards held by individual businessmen, their helpers and freelancers who participate in the city's inpatient and outpatient medical insurance for major illnesses, as well as retired persons holding the "Retired Cadres' Medical Card", cannot be used for basic medical insurance. (2) Participants who only have a social security card or a medical insurance card can use their social security card or medical insurance card to handle the above matters. (3) Participants who have both a social security card and a medical insurance card must bring both cards with them when they first seek medical treatment, and submit them to the staff of the hospital or pharmacy for verification when seeking medical treatment or dispensing medication to determine whether their social security cards are in use. The staff should first use the social security card, if the card has been opened, then tell them that the medical insurance card has been invalidated, and use the social security card in the future; if the social security card has not been opened, and at the same time the medical insurance card can not be used, the cardholder should be told to go to the nearest municipal, district and county medical insurance affairs center to inquire about the reasons. If the card is damaged, a new card will be issued in accordance with the "Operational Guidelines for Replacement of Damaged Medical Insurance Cards". (5) If you find your social security card or medical insurance card damaged during your visit to the doctor, you can go to a neighboring city, district or county medical insurance affairs center for piecemeal reimbursement of the medical expenses incurred on the same day for emergency and outpatient major illnesses, as well as for outpatient medical expenses of the elderly aged 75 or above, by presenting the "Notice of Damage to the Medical Card" issued by the hospital and stamped by the office of the outpatient, and then go to a neighboring city, district or county medical insurance affairs center for piecemeal reimbursement of the medical expenses. 6.
