Procedures for applying for social security cards Any citizen who meets the conditions for applying for a social security card may apply for one with his/her household registration book, his/her ID card and his/her spouse's ID card at the street (township) of his/her place of residence or at the nearest street (township) social security card service station. If, during the application process, the applicant finds that the information on his or her name and civil status number recorded in the public security or labor security departments is inconsistent, or differs from the actual use of the card, he or she should first apply for correction of the information, and then go through the procedure of registering to apply for a social security card. 3. Receiving a Social Security Card After a citizen applies for a social security card, the Municipal Information Service Center will issue a social security card within 30 days from the date of acceptance of the application (except for those who applied before the end of 2001 and those whose information is inconsistent). Any loss to the individual or the social insurance fund caused during the period of time in which the card is not in effect will be the responsibility of the person who lost it. 5. Replacement (replacement) of social security cards After the social security card is damaged or lost, the cardholder can apply in writing for a replacement (replacement) of the card at the nearest district or county social security card replacement (replacement) card service center with his/her account book (the agent with his/her social security card or account book and the applicant's account book), and the staff will check that the information is correct and confirm that there is such person's information in the database of the Municipal Social Security Card Service Center, and review the status of the social security card with that of the applicant. For the time being, the social security cards held by individual businessmen, their helpers and freelancers who participate in the city's inpatient and outpatient medical insurance for major illnesses, as well as retired persons holding the "Retired Cadres' Medical Card", cannot be used for basic medical insurance. (2) Participants who only have a social security card or a medical insurance card can use their social security card or medical insurance card to handle the above matters. (3) Participants who have both a social security card and a medical insurance card must bring both cards with them when they first seek medical treatment, and submit them to the staff of the hospital or pharmacy for verification when seeking medical treatment or dispensing medication to determine whether their social security cards are in use.
