You need a certificate to apply for a children's medical insurance card in a bank (taking Shenzhen as an example);

Children in school (excluding children from low-income families) shall be declared and submitted by the school to the social security agency:

1. Fill in the information and forms required for the insurance application issued by the school.

2, the insured children's birth certificate, residence booklet, ID card, guardian (deep households) family planning service certificate or guardian (non-deep households) family planning certificate copy (original inspection).

3 copies of the guardian's household registration book, ID card and bank passbook (original inspection).
