How to apply for comprehensive burden reduction
1. Eligible participants can go to the neighboring district or county health insurance centers or service points to get the "Comprehensive Reduction of Medical Fees for Urban Employee Health Insurance Participants Application Form" (hereinafter referred to as the "Application Form"), or can be downloaded from the health insurance website.
2. After filling out the Application Form completely, the insured person should have his annual income proved by the seal of the unit he is working in after examination; the individual contributor who pays 14% of the contributions and the co-insured person who has not been re-employed should have his annual income proved by the seal of the relevant functional department of the street office (township or township) in the place where he is located after examination; the retired person does not need to be proved by the seal of the unit in principle but he needs to provide the passbook of the old-age pension.
3. Participants can apply for comprehensive burden reduction at the neighboring district or county health insurance centers or service points with their own valid documents (ID card, household register, etc.), Application Forms, Social Security Cards or Medical Insurance Cards certified with seals.
4. Participants can entrust others to handle the application on their behalf. The delegated person is required to bring along his/her own and the participant's valid documents (ID card, household register, etc.).
Precautions:
1. Before making the comprehensive reduction, the insured person should go to the relevant departments to complete the following types of reductions: reduction of the burden of dialysis personnel, reduction of the burden of the starting line of hospitalization for mentally ill persons, reduction of the starting line of hospitalization for low-income persons in difficulty, the General Union Mutual Assistance Guarantee Scheme, and civil service subsidies.
2. Participants need to complete the Mutual Benefit Scheme subsidy in the trade union department 10 days before they can apply for the comprehensive burden reduction at the neighboring district and county health insurance centers or service points.
