Members go to the "Center" to collect and fill out the "Application Form for Social Insurance Relationship Transfer in Suzhou Industrial Park", and submit relevant materials to the "Center" after being stamped by the transferring unit.

The "center" handles the transfer of members' social insurance relations, corresponding personal account funds and housing provident fund according to the situation.

"Center" will issue a transfer balance sheet for members after handling the transfer formalities of provident fund in the member park.

Members shall provide the following materials when they move from the employer in the park to the employer in Suzhou, and go through the formalities of social insurance relationship and housing accumulation fund transfer:

1. provident fund membership card and ID card

2. The original labor contract termination certificate of the original employer (filed by the labor management department of the park)
