Relevant information to be provided by the unit to repay the provident fund:

1. Completed Application for Repayment of Housing Provident Fund with the official seal of the unit in quadruplicate;

2, complete and stamped with the official seal of the "housing provident fund payment list" in quadruplicate;

3. When the unit handles the deposit of housing provident fund for employees, it is necessary to provide the employee registration form filed by the social security department or the salary approval form and transfer order approved by the competent department to prove the deposit time of employees. Provident fund renewal process:

1. The unit manager shall bring the completed Application Form for Housing Provident Fund Repayment and Detailed List of Housing Provident Fund Repayment to the public counter of the business hall on the first floor of the housing provident fund.

2. After acceptance by the center, the teller of the center will submit the first, second and third copies of the Application for Housing Provident Fund Repayment and the Detailed List of Housing Provident Fund Repayment stamped with the business acceptance seal to the unit manager to handle overdue repayment at the entrusted bank window of the provident fund.
