1. 2. If the photo on the card surface is changed, the cardholder shall go through the card replacement procedures at any branch of the card-issuing bank within the jurisdiction of this Municipality with his ID card, social security card and the original and photocopy of the photo receipt;

3. If the cardholder's personal basic information (including name and ID number) changes, the cardholder should first bring the relevant valid documents (original and photocopy) to the social security agency to which the insured belongs to handle the personal basic information change, and then bring the relevant valid documents (original and photocopy) and social security card to any branch of the city's card-issuing bank to handle the personal basic information change and card replacement procedures.
