How to report the loss of Shanghai social security card

Shanghai written social security card loss reporting method is as follows:

1. After the social security card is lost, the loss reporting person shall report the loss in writing to the Shanghai Social Security Card Service Center (hereinafter referred to as the Service Center) with his/her ID card (the agent shall show his/her social security card or ID card and the principal's ID card).

2. When reporting the loss in writing, the loss reporting person or his agent shall fill in the Report on Reporting the Loss of Shanghai Social Security Card (in triplicate) (see Annex 1).

3. Within 4 hours after the service center accepts the written loss report, the loss report card is frozen. After 30 days of written loss reporting, the loss reporting card will be automatically cancelled.
