1. The process of handling special diseases is as follows:

(1) Carry the information of past medical history to the outpatient doctor, and if the patient meets the conditions, the doctor will fill in the certificate of special diseases for basic medical insurance and the approval form for outpatient treatment;

(2) The approval form for special diseases must be filled in by the physician attending the designated medical institution of second class or above ;

(3) After the hospital has finished the process, the patient or his family members bring the medical history information, medical insurance card, ID card and 2 1-inch photos to the social security office in the place of domicile to apply for, receive and fill out the application form, and the approval form to the local social security bureau for approval and stamp.
