Information required for reinsurance

(1) Provide the completed Application Form for Retirement (Transfer) of Social Insurance Fund for Enterprise Employees or the resignation certificate issued by the original employer (both the application form and the unit certificate shall be stamped with the official seal or personnel seal of the unit);

(2) Show the original personal ID card and submit two copies. If it is a second-generation resident ID card, a copy of the back of the ID card is required;

(3) Submit * * employees' social insurance certificate or Shenzhen labor security card;

(4) Employee individual payment list 1 copy (print the payment records of employees from the first month to the last month, which can be printed in the district social security management office or the business center of the personal service center);

(5) Letter of acceptance from the social security agency in the transferring place (the letter of acceptance shall indicate the name of the account, the full name of the bank where the account is opened and the account number);

(6) If an agent is entrusted to handle reinsurance matters, it shall also provide a power of attorney issued by the principal (the power of attorney shall be signed by the principal and stamped with the official seal or personnel seal of the client's unit according to the fingerprint, or provide a notarized power of attorney), and the original and photocopy of the agent's ID card; If there is a balance in the medical insurance or housing provident fund account, a copy of the customer's passbook or bank card should be provided (note: passbook and bank card that have been opened for less than one month will not be accepted), and an application for transfer should be filled out in duplicate.

Note: If there are still personal account balances of medical insurance and housing provident fund transferred to the employee's personal account, when the employee himself comes to handle the transfer business, he must also go to the bank window to get a bank debit card (the personal account balances of medical insurance and housing provident fund are stored in the card); When the agent comes to handle reinsurance business, the agent provides a copy of the customer's bank passbook or bank card and fills in the transfer application form in duplicate, and then receives another settlement document at the financial window. The social security agency is responsible for remitting the balance of the personal account of medical insurance and housing provident fund to the above account.
