Individuals who meet the application conditions should submit 1 2-inch color ID photo (paper version) and "Application Form for Honor Card of Free Blood Donation in Tai 'an City" to each blood donation point. You can fill in and submit the application form at the blood donation site.

Tai 'an Blood Donation Office will hand over the collected data to Tai 'an Public Transport Co., Ltd. Business Card Printing Center to make Tai 'an blood donation honor card. After receiving the notice of card application, the individual will take the original ID card to each blood donation point to collect it.
