(1) rescue object. Children applying for assistance must meet the following conditions:
1. Clinically diagnosed with the following six congenital structural malformations:
① Congenital malformation of nervous system;
② Congenital malformation of digestive system;
③ Congenital malformation of urinary system and reproductive organs;
④ Musculoskeletal congenital malformation;
⑤ Congenital respiratory system malformation;
⑥ Severe congenital structural malformation of the five senses.
See annex 1 for specific diseases.

(2) The scope of medical expenses subsidies: drugs, beds, examination, examination, radiation, examination, treatment, surgery, blood transfusion, nursing, materials and oxygen infusion.
(III) Subsidy standard: The medical expenses for diagnosis, operation, treatment and rehabilitation in the designated medical institutions of the project will be subsidized after the last year (inclusive) 1 when the child applies for assistance. The project will give a one-time subsidy of 3000-30000 yuan according to the self-funded part of the children's medical expenses after reimbursement. The specific subsidy standards for each child are as follows:
1. If the out-of-pocket part is more than 3,000 yuan (inclusive) but less than 4,000 yuan, the subsidy amount is 3,000 yuan.
2. The out-of-pocket part exceeds 4,000 yuan (inclusive). 75% of the out-of-pocket expenses will be subsidized, and the maximum subsidy amount is 30,000 yuan (inclusive).
For the same child with the same disease to apply for assistance in several times, or for the same child with different diseases to apply for assistance separately, the project will only be funded once.
Second, the rescue process and materials needed
(1) filing an application. The legal guardian of the child applies for medical expenses assistance, fills in the Personal Application Form for Congenital Structural Malformation Assistance Project (Annex 3), submits it to the designated medical institution where the child has received diagnosis, operation, treatment and rehabilitation, and provides the following relevant materials according to the requirements of the application form.
1. Identification materials. A copy of the household registration book and ID card proving the relationship between the child and his legal guardian. If there is a birth certificate, it can be attached with a birth certificate or other original materials to prove the guardianship relationship.
2. Proof of disease and treatment. Original diagnosis certificate of children's diseases, home page of hospitalization, medical records, discharge records and related medical imaging data reports issued by designated medical institutions (or a copy stamped with the official seal of the hospital). Outpatient (emergency) consultation with out-of-pocket expenses, should be provided together with outpatient (emergency) consultation medical records.
3. Proof of family financial difficulties. A copy of the certificate of poor households with low security and low income, or the original poverty certificate issued by the village (neighborhood) Committee.
(2), preliminary examination and information entry. When the designated medical institution receives the application materials, a copy of the certification materials must be checked with the original on the spot and stamped with the seal of the unit. Designated medical institutions shall conduct a preliminary examination of the application materials and submit the materials passed in the preliminary examination to the provincial project management institutions in a timely manner. If the preliminary examination fails, the designated medical institution shall notify the legal guardian of the child, and the legal guardian may supplement the materials to re-apply.
(3) provincial review. Provincial project management institutions regularly organize experts to conduct on-site online review of children's application materials submitted by designated medical institutions.
(4), review and publicity. Provincial project management institutions will submit their children's application materials to the Foundation for review, and the Foundation will regularly publicize the list of children who have passed the review in official website.
(five), issued a receipt. Provincial project management institutions will publicize the list of children without objection and notify the corresponding designated medical institutions. Designated medical institutions issue and guide children's guardians to fill in the receipt of the aided congenital structural malformation rescue project.
The guardian shall submit the receipt, the original compliant medical bill and the bank card or passbook information of the child or his guardian to the designated medical institution. Please refer to the relevant provisions of the receipt for specific requirements.
Designated medical institutions will submit the receipt and related materials submitted by children's guardians to provincial project management institutions, and submit them to the foundation after being audited by provincial project management institutions.
(six), the allocation of disaster relief funds. After reviewing and confirming all the materials and medical bills of the aided children, the Foundation verifies the amount of medical expenses subsidies, allocates the corresponding relief funds to the bank account of the aided children or their guardians, and feeds back the list and amount of the aided children to the provincial project management institutions.
(7) pay a return visit. Designated medical institutions should pay a return visit to the guardians of the aided children within 10 working days after the foundation allocates the aid, fill in the Registration Form for Return Visit of the Children with Congenital Structural Malformation, understand the arrival, rehabilitation and satisfaction of the aided children, and input the return visit information into the project management information system.
Three, accept the declaration of designated medical institutions
Eight medical institutions, including Kunming Children's Hospital, Qujing Maternal and Child Health Hospital, Dali Maternal and Child Health Hospital, Second People's Hospital of Yunnan Province, Second Affiliated Hospital of Kunming Medical University, First People's Hospital of Yunnan Province, First Affiliated Hospital of Kunming Medical University and Yuxi Children's Hospital, are designated medical institutions for the project, and patients can declare to any one of them.
