Proof of applying for birth notarization (with the opinions of the personnel department of the unit where the applicant's file is located and stamped with the official seal); If the above certificates cannot be provided, corresponding certificates issued by other authorities can be provided. If the household registration book directly reflects the relationship between parents and children in the same book, this certificate is not needed;
4. Identity documents of the applicant's parents (domestic residents need to provide ID cards and household registration books, Hong Kong, Macao and Taiwan residents need to provide ID cards and passes, and foreigners need to provide passports such as ID cards, household registration books or passports). If one or both parents die, a death certificate shall be provided;
5. Marriage certificate of the applicant's parents (divorce certificate, divorce agreement or effective court judgment documents are required if the parents are divorced);
6. If the applicant has moved abroad, provide the personal information form of permanent residents or the certificate of cancellation of household registration issued by the local police station where the original household registration is located (indicate the applicant's birthplace);
