Inquiry for renewal registration of nurse qualification certificate of Haozhou City Health Bureau

According to reports, this year’s nurse practitioner qualification review submission materials and registration process will still be implemented in accordance with last year’s regulations. The health administrative departments of various places will be unified to the province for processing. After centralized review, they will be processed at the Provincial Government Affairs Center. The certificate is issued at the window of the Department of Health.

According to the arrangement, all provincial hospitals, Ma'anshan, Tongling, Huaibei, and Huainan will handle it in early November; Bozhou, Bengbu, Hefei, Chizhou, Huangshan, and Suzhou will handle it in mid-November; Wuhu, Lu'an, and Fuyang will handle it in late November, and Anqing, Chuzhou, Xuancheng, Guangde County, and Susong County will handle it in early December. Registration will end before December 25th.

Materials required to apply for renewal of registration

1. One copy of "Nurse Registration Renewal Application Review Form";

2. The applicant's "Nurse Practice Certificate" 》Original and 1 copy;

3. Physical examination certificate within 6 months issued by a local second-level medical institution or above;

4. Proof that the applicant is working as a nurse, Or the original and copy of the employment contract;

4. Processing process

(1) Unit review. Applicants for renewal of registration shall prepare the required materials according to the submission requirements, bind them into a book, and submit them to their unit. Each unit will review the materials submitted by the applicant, and the legal representative of the unit will sign the review opinions on the "Nurse Registration Extension Application Review Form" and stamp it with the official seal of the unit. Medical and health institutions that have been equipped with the "Nurse Practice Registration Online Management Information System (Institutional Version)" will pre-enter the nurse renewal registration information, and submit the summary list and renewal registration materials to the Medical Affairs Section of the Municipal Health Bureau before November 25.

(2) Preliminary review by the health administrative department at the county (city, district) level. Each medical and health institution shall submit the materials of those who meet the registration renewal conditions to the county (city, district) level health administration department, and the county (city, district) level health administration department will review the application materials. Those who pass the review will use the "Nurse Practitioner Registration Online Management Information System (Management Edition)" to pre-enter the information. Submit the summary list of extended registration and extended registration materials for nurses in the county (city) district before November 25th;
