I. Conditions for applying for certification

1. China citizens who have not reached the national statutory retirement age, their domicile, residence (residence permit within the local validity period is required), school location (full-time colleges and universities) (fresh graduates and graduate students only) are in Shangyu District, as well as active servicemen and active armed police serving in our district.

Residents of Hong Kong, Macao and Taiwan who study, work and live in our district must hold valid certificates such as Residence Permit for Hong Kong, Macao and Taiwan Residents, Travel Permit for Hong Kong and Macao Residents to and from the Mainland and mainland travel permit for taiwan residents, which are valid for 5 years. No criminal record, you can apply for the qualification of primary and secondary school teachers in the place of residence and the place where the teacher qualification examination is held. The academic qualifications, other conditions and procedural requirements for applying for teacher qualification are the same as those for mainland (mainland) applicants.

2. Abide by the Constitution and laws, love education, have good professional ethics, and be able to fulfill the obligations stipulated in the Teacher Law.

If a teacher's qualification is revoked, he shall not re-apply for the teacher's qualification within 5 years from the date of revocation; Those who have been deprived of political rights or have received criminal punishment of fixed-term imprisonment or above for intentional crimes cannot apply for teacher qualification.

Postgraduates of education and normal students should pass the assessment of normal students' educational and teaching ability and obtain the teachers' professional ability certificate of normal students, which is within the validity period.

4. Those who have the corresponding academic requirements stipulated in the Teachers Law of People's Republic of China (PRC) and apply for the qualification of internship guidance teachers in secondary vocational schools shall also have the professional and technical qualifications equivalent to assistant engineers or above or the technical level of intermediate workers or above.

5. Hold a certificate of Grade B or above in Putonghua Proficiency Test (Grade B is required to apply for Chinese courses).

6. Have good physical and mental quality, and pass the physical examination in the county-level and above hospitals designated by the teacher qualification accreditation body.

Teachers' qualifications shall be managed in a territorial way. The applicant shall apply to the education administrative department where the school is located (fresh graduates and graduate students only), where the household registration is located and where the residence permit is valid (the residence of urban residents can be handled according to the actual living area on the electronic residence permit) according to the teacher qualification category and the authority recognized by the education administrative department. Applicants can only apply for one teacher qualification in the same year.

Second, certification bodies.

"Senior Middle School Teacher Qualification", "Secondary Vocational School Teacher Qualification" and "Secondary Vocational School Practice Instructor Qualification" are recognized by Shaoxing Education Bureau. "Junior high school teacher qualification", "primary school teacher qualification" and "kindergarten teacher qualification" are recognized by the district, county (city) Education and Sports Bureau.

According to the spirit of the Notice of Shaoxing Municipal People's Government on Further Promoting the Reform of Administrative Examination and Approval Levels in Cities and Counties (Shao [2065438+06] No.35), in order to further facilitate the declaration of teacher qualifications, municipal authorities (senior middle school teachers' qualifications, secondary vocational school teachers' qualifications, and secondary vocational school internship guidance teachers' qualifications) are delegated to districts, counties (cities). Shaoxing Education Bureau has set up on-site confirmation points in Yuecheng District, Keqiao District, Shangyu District, Zhuji City, Shengzhou City and Xinchang County, and each confirmation point is responsible for on-site confirmation, physical examination and issuance of qualification certificates. In addition, Shaoxing University of Arts and Sciences, zhejiang yuexiu university of foreign languages and Yuanpei College of Shaoxing University were set up for confirmation (only for students of our school) to assist Yuecheng District Education and Sports Bureau in the confirmation, physical examination and certification of student applicants.

Third, the appraisal procedure.

(1) Online application time: the first batch, from April 14 to April 28, and the second batch, from June 7 to June 8 (only for education graduate students who have obtained the 2022 teacher professional competence certificates of normal students and normal students).

Applicants log on to China Teachers' Qualification Network (/), enter from the "Teachers' Qualification Network Portal", fill in relevant information according to the system prompts, and apply for online registration. Please refer to the attachment User Manual for Teacher Qualification Applicants -2022 for specific operation. When the online declaration time expires, the system will automatically shut down.

On-site confirmation time: the first batch, working time from May 9 to May 13. The second batch (only for postgraduate students of education who have obtained the professional competence certificates of normal students and normal students' teachers in 2022), during the working hours from June 10 to June 14, the applicants will bring the required materials to the designated on-site confirmation point for confirmation (the online registration confirmation point must be the same as the on-site confirmation point), and those who fail to make on-site confirmation on time will be regarded as giving up automatically.

Confirmation time: 8: 30- 1 1: 00 and 14: 00- 16: 30 every day. Overdue, will not be dealt with.

2. Physical examination

After on-site confirmation, Shangyu Teacher Development Center will organize a unified physical examination, and the time and related requirements will be notified separately. Applicants are required to attend all physical examinations (such as chest X-ray, etc.). Please make arrangements according to your physical condition. Those who fail to take part in (all) physical examination shall be deemed as the applicant's automatic waiver of application.

Applicants for "Senior Middle School Teacher Qualification", "Secondary Vocational School Teacher Qualification", "Secondary Vocational School Practice Instructor Qualification", "Junior Middle School Teacher Qualification" and "Primary School Teacher Qualification" should download and print the Physical Examination Form for Zhejiang Province Teacher Qualification Applicants. Fill in the columns of ID number, name, gender, date of birth, past medical history, etc., and post my photo (the same as the photo uploaded when applying online). Please download, print and fill in the Physical Examination Form for Personnel Applying for Kindergarten Teacher Qualification in Zhejiang Province.

3. Audit findings

Applicants can log in to China Teacher Qualification Network in time to check the status of each application period. Applicants who have passed the teacher qualification certification will be announced by the center.

Obtain a certificate

If the application is approved, please pay attention to the announcement of the authorized institution. Applicants can choose to collect it on the spot or by express delivery (postage is self-care). If there is an accident in the mail, they will be responsible for it. (Applicants who choose express delivery must fill in the mailing address when applying online, and the mobile phone number must be correct and must be in person, and will not change it in the future).

Fourth, confirm the material requirements.

The following materials shall be submitted for on-site confirmation:

1. Original second-generation ID card;

2. Proof of the applicant's identity category:

(1) Apply for confirmation by the place where the household registration is located, and the applicant for household registration in this district shall provide the original and copy of the household registration book (the household registration book only needs to copy the household registration book 1 page and the applicant information page; Collective hukou can also be replaced by the household registration certificate issued by the police station);

(2) If the residence application is approved, submit the original and photocopy of the residence permit within the validity period (or photocopy of the screenshot of the electronic certificate);

(3) To apply for accreditation at the school location, submit the original and photocopy of the student ID card with complete registration information;

(4) Hong Kong, Macao and Taiwan residents apply for recognition, and submit the original and photocopy of mainland travel permit for taiwan residents, which is valid for 5 years and is valid for Hong Kong, Macao and Taiwan residents' residence permit or Hong Kong and Macao residents' pass to and from the mainland within the validity period of our city;

3. Three recent one-inch color photos with a white background and bareheaded front (the photos of official documents should be the same as those uploaded when applying online, 1 Stick a physical examination form, and the other two photos are used for teacher qualification certificate and backup. 4. To apply for the qualification of internship instructor in secondary vocational schools, the original and photocopy of the professional and technical post certificate equivalent to assistant engineer and above or the technical grade certificate of intermediate worker and above shall be provided;

5 Hong Kong, Macao and Taiwan residents applying for the qualification of primary and secondary school teachers shall submit the certificate of no criminal record issued by the relevant departments of Hong Kong Special Administrative Region, Macao Special Administrative Region and Taiwan Province Province. If necessary, the applicant can apply to the Zhejiang Teacher Qualification Guidance Center for issuing relevant letters through the accreditation body in advance;

6. Other supplementary materials.

The following materials will be compared through the national accreditation information system when the applicant applies online. If the comparison is successful, it does not need to be provided on site; If the comparison is unsuccessful, it must be provided on the spot.

(1) Original and photocopy of academic certificate. Foreign academic qualifications should also be submitted to the original and photocopy of the Foreign Academic Certificate issued by the Study Abroad Service Center of the Ministry of Education; Hong Kong, Macao and Taiwan education should also be submitted to the original and photocopy of the education certificate issued by the Study Abroad Service Center of the Ministry of Education;

For the academic qualifications that cannot be directly compared and verified by the national information system, the applicant must also submit the China Higher Education Academic Qualification Report (online application in Xuexin.com), otherwise the academic qualifications will not be accepted as unqualified. Applicants are advised to verify their academic qualifications in Xue Xin. Com, unable to verify the timely application for certification report;

(2) Certificate of Qualification Examination for Primary and Secondary School Teachers. (Printed by the applicant on the National Primary and Secondary School Teacher Qualification Examination Network (ntce.neea.edu.cn));

(3) The original and photocopy of the Putonghua proficiency test grade certificate.
