The materials needed to apply for a medical insurance card are,

1, two recent two-inch bareheaded color photos of the insured;

2. The original and two copies of the insured's ID card;

3. The original and two copies of the household registration book of the insured unit;

4. The original and photocopy of the valid certificate and other relevant materials of other family members except the insured person in the household registration book;

5. When the children of migrant workers are insured, they also need to provide the original and photocopy of their parents' temporary residence permit, the original and photocopy of their household registration book, the proof of going out to work provided by their parents' unit and the proof of labor relations for more than 3 years;

6, belonging to the minimum target, but also need to provide the civil affairs department issued the "urban residents minimum living guarantee Jinling evidence" and a copy of the original, registered to receive the minimum living guarantee materials.

Legal basis: Article 23 of the Social Insurance Law of People's Republic of China (PRC), employees shall participate in the basic medical insurance for employees, and the employer and employees shall jointly pay the basic medical insurance premium in accordance with state regulations.

Individual industrial and commercial households without employees, part-time employees who have not participated in the basic medical insurance for employees and other flexible employees can participate in the basic medical insurance for employees, and individuals pay the basic medical insurance premium in accordance with state regulations.
