Lost medical insurance card how to do

Medicare card replacement process:

1, medical insurance card is lost or stolen, it is recommended that you first call the advisory phone 12333 for the loss of the phone; loss of the materials are as follows:

(1) the original of the second generation of my ID card;

(2) a number of one-inch photographs and so on;

(3) to the nearest social security center to apply for replacement of the card formalities.

2, as soon as possible to the Municipal Social Security Center, Social Security Card Management Section of the acceptance window for the loss, replacement procedures (lost by phone to the counter to handle the period of medical expenses incurred will not be reimbursed);

3, to the Municipal Government Center, first floor of the Agricultural Bank of China to pay for the production of social security cards and the cost of labor;

4, the Medical Insurance Management Section and the card company to do a good job in the card materials and the handover of the card has been made;

5. The recipient of the card receives the newly made health insurance card at the original social security organization with the fee receipt. Medical expenses incurred during the period of 5 working days from the date of counter acceptance of loss and replacement will be reimbursed.

Legal basis: Article 57 of the Social Insurance Law of the People's Republic of China

An employer shall, within thirty days from the date of its establishment, apply to the local social insurance agency for social insurance registration on the basis of its business license, certificate of registration, or seal. The social insurance agency shall examine and approve the application within fifteen days from the date of receipt of the application and issue the social insurance registration document.

If the employer's social insurance registration changes or the employer is terminated according to law, the employer shall, within thirty days from the date of change or termination, apply to the social insurance agency for change or cancellation of social insurance registration.

The market supervision and management department, the civil affairs department and the organization and establishment management organs shall promptly notify the social insurance agency of the establishment and termination of the employer, and the public security organs shall promptly notify the social insurance agency of births and deaths, as well as the registration, relocation, and cancellation of household registration of individuals.
