(2) The insured who has not applied for the second-generation ID card or lost the second-generation ID card shall provide a copy of his account page;

(3) Special people need to provide copies of special certificates, such as disability certificate, work injury certificate, minimum living allowance, etc. After receiving the social security card, if the information on the card surface or in the card is found to be wrong, such as the photo does not match with the person, or the name and voice are different, the cardholder should return the social security card to the social security business processing window through the card issuing unit. If the received social security card is inconsistent with my ID card information, I must hand over a copy of my social security card, ID card or household registration book to the card issuing unit, which will hand it over to the social security card business processing window for a new card. If the cardholder forgets the password of the social security account of the social security card, he/she must bring the social security card and his/her valid ID card to the social security card business processing window to reset the password. If the cardholder forgets the password of the social security card bank account, he must go through the password reset formalities at the business outlets of the cooperative bank with the social security card and his valid identity certificate.

Provincial, prefecture-level human resources and social security departments that are allowed to issue social security cards should formulate clear rules for the application and management of social security cards, and announce them to the public through express delivery in service places and publicity on government websites. The social security card application management program shall include the following contents:

(a) the function and purpose of the social security card;

(two) the issuance, application conditions and application procedures of social security cards;

(three) the scope of use of social security cards (including use restrictions), the use period and the use method;

(four) the procedures for reporting the loss and reissue the social security card after it is damaged or lost;

(five) the rights and obligations of the card issuer, the cardholder and other relevant parties.
