2. Employees are insured for the first time:

(1) Non-Dalian hukou: copy of ID card and receipt of digital photo of the second-generation ID card approved by the municipal public security organ (name and ID number must be filled in the receipt).

⑵ Dalian hukou: In addition to the copy of ID card and the receipt of the digital photo of the second-generation ID card approved by the municipal public security organ (the name and ID number must be filled in the receipt), one of the following documents must be attached;

(1) notice of transfer of labor and personnel department and its copy;

(2) the letter of assignment of college graduates and their copies;

(3) the labor department recruitment form and a copy;

(4) the military administrative letter of introduction and a copy;

⑤ Relocated households or long-term residents must provide household registration books and copies;

⑥ Blue printed household registration book and its copy.
