The information you need to bring with you to apply for a health insurance card for your baby:

1. Go to your community or health insurance center to get the Registration Form for Urban Residents' Basic Health Insurance and fill in the relevant information;

2. Bring the original hukou book and a copy of the hukou book (a copy of the main page of the household and the baby's personal page);

3. The original hukou book, a copy of your hukou book or ID number;

4. A group photo of the guardian and the participant, 2-inch photo, 4 pieces. (Each region has different requirements, depending on the implementation rules of the local urban residents' basic medical insurance).
