The social security card has not been activated for five years.

The social security card is not activated without any loss and will not be invalidated, but the medical insurance card will be restricted. If the social security card is not activated, you only need to go to the bank with a valid certificate to activate it, or you can let your family activate it instead of yourself. You need to activate it with a power of attorney and valid documents from both parties.

If the activation time is specified within 6 months from the date of issuing the card, the original medical insurance card of the insured who has not been activated in the bank will be automatically invalidated, but the social security card does not need to be re-applied. After the bank is activated, the amount in the original medical insurance card account will be transferred to the medical insurance card. The insured person needs to activate the social security card within six months after receiving it. If the original medical insurance card is not activated, the use will be restricted. The following personnel provide methods to activate the social security card.

First, the insured person needs to hold his social security card and the original identity certificate to the corresponding bank outlets to activate the social security card, and the medical insurance account will return to normal after activation.

Two, the use of social security cards in designated hospitals to directly enable social security functions. When the insured person uses the social security card for the first time in a designated medical institution, he can directly activate the social security function by simply registering for outpatient service or hospitalization.

Third, when the insured uses the social security card for the first time in the designated medical insurance pharmacy, he inserts the original medical insurance card, and then inserts the social security card as required to activate the social security card.

Sixteenth provincial, municipal human resources and social security departments that are allowed to issue social security cards shall formulate clear rules for the application and management of social security cards, and announce them to the public through express service places and publicity on government websites. The social security card application management program shall include the following contents:

(1) Functions and uses of social security cards.

(two) the issuance, application conditions and application procedures of social security cards.

(three) the scope of use of social security cards (including use restrictions), the term of use and the method of use.

(four) the procedures for reporting the loss and reissue the social security card after it is damaged or lost.

(five) the rights and obligations of the card issuer, the cardholder and other relevant parties.
