The following materials shall be provided for completing the formalities of endowment insurance:

1. The original and photocopy of my ID card, and the original and photocopy of the household registration book (home page, personal page and change page) are three copies each;

2. The original and two copies of the labor contract (employment contract) being performed;

3. The original file of the applicant provided by the employer (the original file of the applicant is returned to the employer after being verified by the archiving agency);

4. Debit card or passbook of personal settlement account of Postal Savings Bank.
