3. Take the ID card, the medical insurance card, the certificate of canceling the labor contract provided by Dalian, and the roster of medical insurance personnel (with official seal) provided by Dalian to cancel the medical insurance card at the 4th floor of the Social Security Center on129th Street, and take out the money from the medical insurance card.

4. Take the social security admission notice, new labor contract and ID card to the 3rd floor of Social Security Center on129th Street to handle the transfer form.

Just go back to Anshan with the transfer form.
