(II) If the basic identity information (including name, citizen's ID number, gender, nationality and other basic identity information) changes, it is necessary to go through the formalities for changing personal data at the social security agency first, and after receiving the Confirmation Form for Change of Personal Data issued by the social security agency, the cardholder shall apply for card replacement at the service outlets of the social security card center with the original and photocopy of his ID card and the Confirmation Form for Change of Personal Data. Seven working days after receiving the new social security card, I will take the card replacement business confirmation form and the original ID card to the original processing point to receive the new card, and at the same time hand over the old card to the business processing point and send it to the social security card center service network for unified destruction.
