The first step

Registration for social insurance

Step 2

Participation of Unit Personnel

1. Hold the original copy of Beijing Social Insurance Unit Information Registration Form and the copy of Authorization for Payment by Special Entrusted Collector of the Same City of Beijing.

2. Download the "Five Insurance Units and One Unit Ordinary Unit Version" from the website of Chaoyang District Social Security Center, complete the operation of entering the unit and personal information in the software, and then make an information report (the file exported from the USB flash drive is in the format of DAT)

3. Submit forms and information for newly enrolled persons: Beijing Social Insurance Personal Information Registration Form" (Beijing Labor and Social Insurance Form 20), "Beijing Social Insurance Participants Increase Form" (Beijing Labor and Social Insurance Form 6); you need to provide the first page of the household register, the page of the person and a copy of the ID card; 2 one-inch color photographs without a crown.

Step 3

Receiving the social insurance registration card and password

1. Receiving the social insurance certificate: The participating unit will receive the social insurance certificate and password with the "Beijing Social Insurance Registration Card" after the social insurance premium is paid. After paying the social insurance premiums, you can get the certificate with the original copy of "Beijing Social Insurance Unit Information Registration Form" and a copy of the latest month's medical and four insurance payment receipt ("Same City Special Entrusted Payment Receipt") or a copy of the "Social Security Center's Financial Receipt".

2. Receive the password: hold the original Social Insurance Registration Certificate and receive it at the window No. 29 of the Registration Department and Official Seal

Step 4

Receive the Medical Passbook for Newly Unified Personnel

After three months of payment of contributions by newly enrolled personnel of medical insurance, the unit will bring the Social Insurance Registration Certificate, the letter of introduction of the unit, and the original ID card of the person who handles the case to the Bank of Beijing, Dongdaqiao Sub-branch to to receive the medical insurance passbook.
