I lost my social security card in Chengdu. What do I need to bring in the replacement process?

Report the loss first and then reissue it:

1. Telephone loss reporting: Call 12333, provide ID number or social security code and 8-digit inquiry password, and report the loss through the self-service voice system.

2. Report the loss in writing: fill in the loss report form at the social security card service outlets.

3. Self-help loss reporting: self-help loss reporting is carried out on the electronic touch screen of self-help terminals set up by social security card service outlets and designated medical institutions.

Re-submit the required materials:

1. Apply to any social security agency in Chengdu for a replacement card with my original ID card or household registration book. If you find someone to do it for you, you must also provide the original ID card of the agent.
