How to apply for social security when giving birth to a child in Chengdu?

Before production, the insured shall pay the maternity insurance premium in full and continuously for 12 months (i.e. after the birth of 13 months), and give birth in designated social security medical institutions (including designated social security medical institutions in different places) in accordance with the relevant provisions of the family planning policy and the Marriage Law. Within 90 days from the date of birth, the insured or his agent shall bring relevant information to the social security agency of the insured place for reimbursement (when reimbursing). Those who do not meet the above conditions cannot enjoy maternity insurance benefits.

The municipal female insured person shall provide the following information for reimbursement of maternity insurance: fill in the Approval Form for Maternity Insurance Benefits in Chengdu in duplicate, ID card, original hospitalization expense document, discharge certificate, birth index, original ID card of the handler, baby birth certificate or other medical certificates, marriage certificate and personal settlement passbook (individual insured person shall provide).
