American citizens born overseas apply to the local American embassy/consulate. The DS- 1 1 form can be picked up in USPS store, or printed online, or filled in manually by printing a blank form. Some inapplicable items can be left blank or written as N/A. Three. Documents submitted with the form
Citizenship certificate: children born in the United States provide birth certificates, babies born overseas by American parents provide citizenship certificates, and naturalized people provide naturalization certificates (commonly known as citizenship papers). This certificate is submitted with the form and will be sent back when the passport is ready.
Photo: driver's license, military officer's card and other documents with photos and names, and copies of the front and back. Copies are submitted with the form and will not be returned. The original needs to be shown to the office staff for inspection and returned after reading.
