The basic medical insurance fund payment form (attached table 10) is in triplicate.

According to the spirit of the document "Notice on Issues Related to Payment of Late Payment Fee for Social Insurance Fee Arrears by Employers" (Beijing Social Security Bureau [2011] No.39), if the social insurance premium is paid back for more than three months, the employer needs to provide proof of labor (employment) relationship with the insured during the payment period and proof of salary income.

If you have a reservation number:

1 On the agreed date, the employer shall go to Room 324, Audit and Supervision Department, Chaoyang Social Security Center with the required materials for supplementary payment.

2 The employing unit shall handle overdue business at the collection counter on the first floor of the handling hall of Chaoyang Social Security Center with the filing form and related materials issued by the audit and supervision department.

The employer shall go through the payment formalities at the financial collection counter with the filing form. After payment, the financial department should mark the registration form accordingly to show that the formalities have been completed.
