How do children in Zhengzhou apply for social medical insurance?

1. Residents apply for insurance in the community where their registered residence is located. The information is as follows:

Original and photocopy of ID card and household registration book;

One-inch color bareheaded photo;

Business card printing cost 12 yuan/person;

Medical insurance premium: 18 years old or older 180 yuan, 18 years old or younger, 30 yuan. The subsistence allowance only pays commercial insurance premiums, not medical insurance premiums.

2, community workers input information, input confirmation form and paste photos, residents check the information and sign for confirmation;

3. Residents pay card fees and medical insurance fees at any business outlet in Bank of Zhengzhou (if you need to exchange invoices, please take the bank receipt to the Finance Department II of the Municipal Medical Insurance Center to issue them);

Registered insurance

Urban residents take the family as the insurance unit for bundled insurance, and all family members within the coverage of urban residents' medical insurance are jointly insured. Family members refer to all the people in the household registration book.

Residents shall go through the insurance registration formalities in the community where their household registration is located, and carry the following materials:

2. The original household registration book of family members and a copy of the household registration book, and residents with collective accounts hold a copy of the collective account;

3. One-inch bareheaded color photos (photos will not be handed in for the time being, and I will post photos after paying the medical insurance card fee);

4, residents should provide proof materials to enjoy the minimum living allowance.

Family members who have not participated in residents' medical insurance under any of the following circumstances shall provide relevant certification materials:

1, to participate in the medical insurance for urban workers, provide a copy of the medical insurance card (this) or a copy of the personal recent payment receipt;

2. If attending a junior college or technical secondary school, provide a copy of the student ID card, a copy of the admission notice or a school certificate issued by the school;

3, migrant workers, provide proof of the employer or a copy of the labor contract signed;

4 long-term residence in the field, provide proof of the local police station or a copy of the temporary residence permit;

5. If there are retired cadres in family members, provide retirement certificates.

6 unemployed people who did not participate in the residents' medical insurance during the period of receiving unemployment benefits shall provide the original and photocopy of the certificate of receiving unemployment benefits.
