Treatment conditions:

Maternity medical insurance in other cities in China (excluding Hong Kong, Macao and Taiwan) shall be paid by individuals in advance. Within/0/2 months from the date of expenses, prepare the following materials and submit them to the affiliated social security bureau for review and reimbursement, which will be approved and reimbursed by the affiliated social security institution according to regulations.

1. Family planning certificate (original 1), certificate of insured unit, maternity manual or outpatient medical records of pregnancy test (copy 1, original test), marriage certificate (copy 1, original test) and birth medical certificate (copy/kloc-0)

2. Reimbursement of medical expenses for childbirth in hospitals outside the city shall be based on the family planning certificate (original 1), the certificate of the insured unit, the maternal handbook or the medical records of the pregnancy check-up clinic (copy 1, the original), the marriage certificate (copy 1, the original) and the birth medical certificate (copy 1, the original).

3. Supplementary information shall be provided for the reimbursement of medical expenses for family planning operation outside the city: marriage certificate (1 copy, original test), birth control operation certificate (1 copy, original test), outpatient medical records (1 copy, original test), family planning certificate (only fallopian tube recanalization or vas deferens) and hospitalization medical records (hospitalization medical records are required).

Third, the maternity insurance reimbursement process: the insured person of maternity insurance applies for reimbursement within one year from the date of the occurrence of maternity medical expenses.

1, the insured person who participates in maternity insurance will be reimbursed at the insured branch;

2, branch staff to review the reimbursement data provided by the insured, and decide whether to accept; If the information is incomplete, a notice of one-time supplementary materials shall be issued;

3, the staff to sort out the acceptance materials, according to the relevant provisions of the medical insurance fund to pay the audit step by step;

4. After the step-by-step audit, the insured person signs to confirm the reimbursement amount and submits it to the financial department for step-by-step audit.
